The game itself came out in 1990 on NES and Game Boy at the same time. Mario is not a plumber here. He is a virologist working at Mushroom Kingdom Hospital, tossing colored Megavitamin capsules into a bottle to wipe out three viruses. Red Fever, blue Chill, and the yellow one just called Weird. Nintendo very deliberately called them vitamins and not pills, and even then some parents still complained about medicine in a kids game.
It was produced by Gunpei Yokoi, the legend behind the Game Boy itself, and here is the detail that always gets me. The music was written by Hirokazu Tanaka, the same composer who did the soundtracks for Metroid and Kid Icarus. That Fever track playing in the background became one of the most recognizable pieces of music Nintendo has ever made.
