But here's why it happened. The slot official gets screened by KAT moving into his sightline right as the push occurs. Wemby's body and KAT's body together shield him from seeing that left arm. At the same time, the lead is locked in on the Johnson and KAT physicality in his primary coverage area, and the trail is locked in on the on-ball competitive matchup with Shamet.
Every official had a legitimate reason to miss it. That doesn't make it acceptable. It just explains how three professionals can all be in the right spot and still not see the same play.
