Dr. Stacy Sims OfficialYour bones don't just start failing at menopause. The cascade starts years before your last period, and most women don't find out until a wrist fracture at 50 tells them they were already decades behind.
In this episode, I sit down with Dr. Tracey Clissold, founder of OSTEO-GAINS and a bone health and jump training specialist, and Dr. Jocelyn Wettstein, orthopedic surgeon and women's musculoskeletal health researcher at Duke University, to cover everything active women need to know about building and protecting bone across the female lifespan. We get into why common exercise approaches aren't enough, what actually moves the needle on bone density, and how to stack exercise, nutrition, and hormonal tools together for a skeleton that stays robust well into your 60s and beyond.
This is a science-first episode for active women who want to stay strong, resilient, and structurally robust across every decade of life.
We cover: • Why bone loss accelerates before menopause (and what to do now) • Can you reverse osteopenia and osteoporosis with exercise after 60? • Why walking isn't enough for bone density in women over 40 • Resistance training vs HRT for bone density: what the research says • How declining estrogen affects muscle quality and power output • Creatine monohydrate for women: bone, muscle, and brain benefits • GLP-1 medications and bone loss: what active women need to know • Why cycling builds your quads but won't protect your bones

Dr. Stacy Sims explains why heavy lifting in the 3-5 rep range is specifically what women over 40 need to preserve type 2 muscle fibers, drive neuroplasticity, produce lactate as brain fuel, and protect against Alzheimer's risk as estrogen declines. This is not about aesthetics - it is about staying strong, cognitively sharp, and independent for decades.

Dr. Stacy Sims breaks down why heavier loads with lower reps preserve the power base that predicts longevity and independent function, and why the lactate produced by those type II fibers becomes a preferred fuel source for the brain as estrogen declines. If you've been told calories in, calories out is all you need to shift menopausal weight, this clip explains exactly why that model fails female physiology and what to replace it with.

This discussion focuses on why every woman’s menstrual cycle can vary cycle to cycle, including more anovulatory cycles where you may still bleed without progesterone. Stacy Sims breaks down how tracking your own pattern helps you spot predictable low days, like feeling rough around day 18, so you stop blaming fitness and start adjusting training load to match hormone flux. When you back off on the hard days and push more on the days you feel great, you create a bigger training stress and improve faster. This clip explores how prioritising the gut microbiome, especially with more fibre and a wider variety of colourful plants, supports body composition and strength as hormone exposure changes and gut diversity can drop in late perimenopause. Stacy Sims breaks down how dietary fibre drives short chain fatty acids and why that matters for inflammation, joints, and bone turnover, plus where GLP 1 medications may help or hurt depending on muscle loss and training. Before you spend another dollar on peptides, there are two free interventions that will do more for your energy metabolism, hormone balance, and cognitive function than any injectable and most women are getting both of them wrong. This clip gives you the science and the protocol.

#DrStacySims #WomenAreNotSmallMen #WomensHealth #FemaleAthletes #ExerciseScience #Hormones #StrengthTraining #MenopauseFitnessThe Eating Window That Actually Works for Women Over 40 (Its Not What You Think)Dr. Stacy Sims Official2026-03-28 | In this conversation, I discuss how to fuel your day around training, why I don’t recommend fasted workouts for women, and how meal timing supports hormones, metabolism, and sleep. I explain the circadian rhythm differences between women and men, the morning cortisol peak, and why women benefit from eating within 30 minutes of waking to protect lean mass. This segment looks at protein distribution across the day, smaller regular meals, and trimming a little from dinner to support sleep hygiene. I also cover time-restricted eating - why late windows (e.g., noon to evening) don’t show benefits in the data, and why earlier windows do, plus how men and women respond differently. We touch on CGM insights and when a small pre-bed top-up may help if low nighttime glucose is waking you.

#DrStacySims #WomenAreNotSmallMen #WomensHealth #FemaleAthletes #ExerciseScience #Hormones #StrengthTraining #MenopauseFitnessWhy Women Were Taught to Be Proud of Losing Their Period (And How Its Destroying Female Athletes)Dr. Stacy Sims Official2026-03-26 | In this conversation, I discuss how to decide when to push or pivot your workout across the menstrual cycle without leaning on heart rate variability. HRV drops with progesterone after ovulation, so it’s useful to confirm ovulation, but it doesn’t predict today’s performance. I talk through the 10-minute rule, your finite stress budget, and why three off days in a row can be appropriate when your body says so. We also unpack the myth that high-intensity resistance training disrupts women’s cycles; the real driver is underfueling, which leads to low energy availability and hormone disturbances - not the intensity itself. This clip is for midlife women especially in perimenopause who want science-based decisions around training and recovery.

#DrStacySims #WomenAreNotSmallMen #WomensHealth #FemaleAthletes #ExerciseScience #Hormones #StrengthTraining #MenopauseFitnessWhy Eating Less and Training More Makes Women Lose Muscle and Gain FatDr. Stacy Sims Official2026-03-25 | In this conversation, I discuss why resistance training needs to be your foundation as you age, with endurance as the “soul food,” not the main course. I explain how heavy, power-focused lifts (3-5 reps, 3-5 sets) build muscle and bone and improve balance and reaction - key for long-term health and independence.
This clip also looks at under-fueling and RED-S in female endurance athletes. I talk through how low energy availability downregulates hypothalamic kisspeptin, suppresses luteinizing hormone, and turns down thyroid and resting metabolic rate - driving conservation, loss of lean mass, and weight gain despite more training. I share first steps to climb out: fuel before and after sessions, add 200-500 calories, use polarized training, and expect early scale changes from glycogen and water before body composition, strength, and sleep improve.

This clip cuts through the noise on two of the most underexplored conversations in women's health: what combined oral contraceptive pills actually do to your training performance and inflammatory load, and whether MHT is really the cornerstone of long-term health or whether resistance training, protein, and gut diversity are doing more than most women realize. If you've been told MHT protects your bones and brain without anyone mentioning what the research actually shows about lifestyle's role, this is the conversation you need.

#DrStacySims #WomenAreNotSmallMen #WomensHealth #FemaleAthletes #ExerciseScience #Hormones #StrengthTraining #MenopauseFitnessWhy Are Women Chronically Under-Eating Protein - and How Much Do You Actually Need?Dr. Stacy Sims Official2026-03-21 | In this conversation, I discuss how much protein women need each day and why pairing carbohydrate with protein supports lean mass. I talk through daily targets (1.8-2.3 g/kg), practical meal spacing (35-45 g at meals; 15-20 g for snacks), and how to bookend hard training with protein. I explain anabolic resistance with age and why consistent, higher-quality doses matter. For plant-based women, I share strategies to hit these targets eating more total volume, using foods like nuts, seeds, sprouted grain breads, edamame, green peas, quinoa, black rice, and tempeh and what I carry when traveling, including hemp and brown rice protein, or a whey isolate if needed.

I outline what MHT can support - bone density and a slower rise in visceral fat via anti-inflammatory effects and where nutrition like omega-3s also plays a role. Then I get into what carries you through peri- into postmenopause: keep strength to maintain lean mass, include some jump work for bone health, and add intensity for cardiovascular and brain health. Whether you choose MHT or not, the training principles don’t change and late 30s to 40s are a key window to build what you’ll need later.

I talk through practical ways to make fruits, vegetables, and whole grains the base of the plate - think a Japanese-style bowl with meat as the accent and simple tactics like blending extra greens into sauces. I outline realistic protein targets by lifestyle, show how daily totals add up when you combine sources, and compare plant and animal options. We map a fully plant-based day that reaches higher intakes, and I explain when pea or whey isolates make sense especially with heavier training. I also highlight the 80/20 rule for real life and why adequate protein supports recovery and immune function when seasons shift and germs are everywhere.

#DrStacySims #WomenAreNotSmallMen #WomensHealth #FemaleAthletes #ExerciseScience #Hormones #StrengthTraining #MenopauseFitnessWhy High Intensity Training Works Better Than Zone 2 for Women in PerimenopauseDr. Stacy Sims Official2026-03-16 | 📩 Join my newsletter for updates, tips, and free resources 👉 drstacysims.com/generic_optin
In this conversation, I discuss how to polarize training in perimenopause: true high-intensity intervals (80-85% heart rate with variable recovery to total ~18-20 minutes in-zone) and sprint intervals (≤30 seconds all-out with 2-5 minutes easy) to drive muscle quality, insulin sensitivity, and mitochondrial function. I explain why zone 2 can be “soul food” but shouldn’t be your weekly backbone, how to avoid the moderate-intensity trap common in group classes, and why 2-3 quality intensity sessions beat lots of steady cardio for midlife women. This clip also covers pairing short heavy lifting (low reps, full recovery, ~25-30 minutes) with brief sprints in the same session, spacing for recovery, and practical ways to shift from sympathetic to parasympathetic - sleep first, simple breath and mobility work, and training that leaves you worked, not exhausted.

This clip explores why menstrual cycle and performance is highly individual, and why tracking your own cycle can help you train smarter. Stacy Sims breaks down what earlier research suggested about the low hormone phase versus post ovulation changes in core temperature, metabolism, and neurotransmitters, then explains what newer research shows: cycles can vary, including more anovulatory cycles, so your best plan is to identify your personal patterns. I explain how falling estrogen lowers myosin responsiveness, why the nervous system needs heavy loads to recruit more fibers, and where protein becomes essential to back your lifting. I also outline how long, moderate training elevates cortisol and can drive lean mass loss, and why prioritizing heavy strength and high intensity helps preserve power and speed with age. #DrStacySims #WomenAreNotSmallMen #WomensHealth #FemaleAthletes #ExerciseScience #Hormones #StrengthTraining #MenopauseFitnessHow to Use Creatine for Muscle, Brain Power & Better RecoveryDr. Stacy Sims Official2026-03-09 | Creatine is the most researched supplement in existence, yet the conversation has almost exclusively been built around men.
In this video, I get into what creatine actually does inside the female body, how it supports everything from muscle contraction and bone density to brain metabolism and gut health, and why the side effects most women fear are almost entirely a product of the wrong dose, the wrong source, and advice that was never designed for us in the first place.
I also break down the three distinct use cases for creatine, whether you are using it for performance, general health, or to support your brain through sleep deprivation and cognitive fatigue, because the dose and approach are completely different for each.
If creatine has felt confusing, overwhelming, or just not relevant to you, this video changes that.
You’ll learn: • Why creatine was recommended to every male athlete but almost never to women • How creatine fuels the brain, gut, heart, and muscle at a cellular level • Where the bloating myth actually came from and why it does not apply to you • What intra-muscular water retention really is and why it creates muscle definition • Why the source and purity of your creatine matters more than the brand • The three different dosing approaches for performance, health, and brain support • What the latest research says about creatine for perimenopause and menopause • How higher doses of creatine support brain function during poor sleep and shift work • Why your creatinine levels may spike on blood work and what to tell your doctor • How long it actually takes for creatine to fully saturate your tissues • Why creatine is vegan and vegetarian friendly even though it is derived from animals • Whether preloading creatine ahead of a stressful or sleep-deprived period makes sense

I talk through quick options for the 5 AM crowd - small carb plus protein like protein coffee, a few spoonfuls of overnight oats or yogurt, or protein water and when to top up if breakfast was more than about 90 minutes ago. I explain why women benefit from eating within roughly 45 minutes after training: exercise is a catabolic signal, and timely protein with some carbohydrate supports euglycemia and tells the hypothalamus to start repair and rebuilding. This segment also looks at training near bedtime and sleep: intense sessions late at night can disrupt core temperature drop, so if evenings are your only window, front-load most of your food earlier and finish with protein plus a little carbohydrate. We also touch on chronotype and shift work, where consistency and smart refueling help protect recovery in midlife.
