The True ending is what you get for reaching the end with an overall A ranking and not fulfilling the other requirements for the Happy ending. More on that later.
The stage is just the same fight against the Supreme God as the Normal ending. But more is revealed on the red and black insects. They're actually beings from another world where the gods used to reside, and apparently did something so bad there that it made them hate the gods so much they hunted them all the way to another world to finish them off. Their proper name are "God Hunters", which sounds pretty badass. Dorothy was created by the gods with the hopes of being able to combat them and protect the gods, which is why they wanted her so badly, they were scared of the God Hunters.
Dorothy says there's no way we could defeat them, and so transport both herself and them into another world, thereby saving Fiel and this world. Leon and Almira survive in this ending, but we lose Dorothy. I find it interesting that this ending is called the "True" ending and the hardest one to get is the "Happy" ending. So it leads me to believe this would have been like the canon ending that would be continued off of if this game ever got a sequel.
