𝗦𝗼𝘂𝘁𝗵𝗲𝗿𝗻 𝗔𝗶𝗿𝘄𝗮𝘆𝘀 𝗙𝗹𝗶𝗴𝗵𝘁 𝟵𝟯𝟮 was a domestic charter flight from Kinston to Huntington, operated by a McDonnell Douglas DC-9 (Reg. N97S) on 𝗡𝗼𝘃𝗲𝗺𝗯𝗲𝗿 𝟭𝟰, 𝟭𝟵𝟳𝟬.

The plane departed Kinston at 18:38 local time carrying players and fans of the Marshall University Football Team based in Huntington. Following an uneventful flight, the crew contacted Huntington Approach Control at 19:23 and was cleared for a runway 11 localizer approach. At 19:34 the flight passed the Outer Marker and they cleared to land. In weather conditions of mist and light rain with low clouds, the plane descended below MDA and struck trees on a hill, approx. 1 mile short of the runway.

The plane then crashed and caught fire. The plane was totally destroyed by impact forces and a post crash fire and none of the 75 occupants survived the crash. Among the 75 occupants were 36 players, 9 coaches and administrators, 26 fans and 4 crew members. At the time of the accident, there were scattered clouds at 300 feet, broken overcast at 500 feet and solid overcast at 1,000 feet.

𝗣𝗿𝗼𝗯𝗮𝗯𝗹𝗲 𝗖𝗮𝘂𝘀𝗲:
"The descent below Minimum Descent Altitude during a non precision approach under adverse weather conditions, without visual contact with the runway environment. The Board has been unable to determine the reason for this descent, although the two most likely explanations are: a) improper use of cockpit instrument data; or b) an altimetry system error."

The flight was being operated by a Boeing 707 (Reg. HK-2016) on 𝗝𝗮𝗻𝘂𝗮𝗿𝘆 𝟮𝟱, 𝟭𝟵𝟵𝟬. The plane departed Medellín at 15:08. When arriving near New York, the plane had to enter 3 holding patterns. The first for 19 minutes over Norfolk, the second for 29 minutes over New Jersey, and the third pattern over the CAMRN intersection for 29 minutes. At 20:44:43, while holding at CAMRN for 26 minutes, the New York ARTCC radar controller advised AVA052 to expect further clearance at 21:05. At that moment the crew advised ATC that they could only hold for 5 more minutes and that their alternate Boston couldnt be reached anymore due to the low state of fuel. The flight left the holding pattern at 20:47 and the crew contacted the New York TRACON who guided AV052 for a runway 22L ILS approach. On finals, at 21:15 the crew contacted JFK Tower and they were cleared to land 4 minutes later. Due to the bad weather (300 feet ceiling, 400 m visibility, RVR of 2,400 feet and wind shear of ca. 10 knots) the crew had to carry out a missed approach at 21:23. ATC vectored the crew for another approach. About 21:32, at 12 miles SE of JFK Airport, engines no. 3 and 4 ran down. Shortly afterwards followed by the remaining two. At 21:34, heading 250° and flaps at 14° and gear up, the plane impacted on a hillside in a wooded residential area on the north shore of Long Island. The starboard side of the forward fuselage impacted and fractured the wooden deck of a residential home. 𝗣𝗿𝗼𝗯𝗮𝗯𝗹𝗲 𝗖𝗮𝘂𝘀𝗲: The failure of the flight crew to adequately manage the airplanes fuel load, and their failure to communicate an emergency fuel situation to air traffic control before fuel exhaustion occurred. Contributing to the accident was the flight crews failure to use an airline operational control dispatch system to assist them during the international flight into a high-density airport in poor weather. Also contributing to the accident was inadequate traffic flow management by the FAA and the lack of standardized understandable terminology for pilots and controllers for minimum and emergency fuel states. The Safety Board also determines that windshear, crew fatigue and stress were factors that led to the unsuccessful completion of the first approach and thus contributed to the accident.$
