I forgot to mention, but for the sword+hammer charge attack, if you're too close to him, roll when you see the sword raise slightly, angle it to his backside (preferably on his left, i.e. your right). For the 2x light sword + stab combo up close... primarily, you want to avoid being caught in that to begin with. I've tried many directions, but it seems very dependent on your roll timing. Many directions have worked, but none consistently.

For healing opportunities: You can heal directly after he jumps back, you can heal while he's doing the four hit combo if you're far enough away, same with sword+hammer charge. If you're pretty far from him you can also heal, but you might eat a throwing dagger. Don't panic and react accordingly, it doesn't do much damage. Most opportunities to attack you can also choose to heal, of course.
