Replacing my one ton CM chain hoist with a massive six ton Yale hoist I got off Craigslist. I perform all my own stunts in this video.
Partial list of safety violations in this video.
1.) Failure to maintain 3-point contact on the ladder at all times.
2.) No fall arrest harness.
3.) No approved safety foot ware. (They are composite toe Brunt Workwear.)
4.) No side guards no glasses. Please bring glassed to HR-Safety Committee meeting so that we may confirm proper eyewear.
5.) No face guard while grinding and/or cutting with the abrasive wheel.
6.) No safety guard on grinder.
7.) No posted hot work permit.
8.) No fire suppression gear directly adjacent to worksite.
9.) No fire watch.
10.) Improper use of pallet for lifting, approved pallet required.
11.) Weight of trolly clearly exceeds OSHA guidelines for manipulation by an mature employee*.
12.) Use on non-approved lifting clamps.
13.) Expired sling / no sling safety audit on file.
14.) Inappropriate us of "milk crate" an a work holding surface.
15.) No current forklift operator certification.
16.). No material safety data sheet provided for solvent or petroleum products used.
17.) No engineering report approving use of I-beam as fit for purpose (@ min.1.5X rated capacity).
18.) No certification from approved, external testing authority on chain hoist condition.
19.) No safety or operational signage on hoist.
20,) Use of language or phrases the are triggering to other workers, i.e. "dogmeat"
21.) Work plan did not include approved and required scaffolding with railings.
22. No hardhat or hearing protection.
