๐๐ฒ๐น๐๐ฎ ๐๐ถ๐ฟ ๐๐ถ๐ป๐ฒ๐ ๐๐น๐ถ๐ด๐ต๐ ๐ญ๐ญ๐ฐ๐ญ was a scheduled domestic passenger flight from Jackson to Salt Lake City with a stopover in Dallas. The flight was being operated by a Boeing 727 (Reg. N473DA) on ๐๐๐ด๐๐๐ ๐ฏ๐ญ, ๐ญ๐ต๐ด๐ด.
Flight 1141 left DFW's Gate 15 at 08:30 and was instructed to taxi to runway 18L. When first in line for takeoff (at 08:59) the flight was cleared for takeoff. The takeoff was uneventful until the airplane reached the rotation phase (at 154 knots, 6,017 feet down the runway). As the main gear wheels left the ground, the plane began to roll violently, causing the right wingtip to contact the runway (1,033 feet after lift-off), followed by compressor surges. The plane continued and struck the ILS localizer antenna array 1,000 feet past the end of runway 18L.
After impacting the antenna installation, the plane remained airborne for an additional 400 feet, then struck the ground, traversed a ground depression and slid sideways until it came to rest near the airport perimeter fence, 3,200 feet from the runway end. Parts of the plane had separated in the slide and a fire had erupted in the right wing area, quickly engulfing the rear, right side of the plane after it came to rest. 12 passengers and 2 crew members were killed. The plane was destroyed.

"(1) The captain and first officer's inadequate cockpit discipline which resulted in the flight crew's attempt to takeoff without the wing flaps and slats properly configured; and (2) the failure of the takeoff configuration warning system to alert the crew that the airplane was not properly configured for the takeoff.
Contributing to the accident was Delta's slow implementation of necessary modifications to its operating procedures, manuals, checklists, training and crew checking programs which were necessitated by significant changes in the airline following rapid growth and merger.
Also contributing to the accident was the lack of sufficiently aggressive action by the FAA to have known deficiencies corrected by Delta and the lack of sufficient accountability within the FAA's air carrier inspection process."

On ๐๐ฒ๐ฏ๐ฟ๐๐ฎ๐ฟ๐ ๐ฎ๐ฒ, ๐ฎ๐ฌ๐ฌ๐ฐ, a Beechcraft Super King Air (Reg. Z3-BAB) departed Skopje on an official flight to Mostar, carrying 2 pilots and 7 passengers, among them Boris Trajkovski, President of the Republic of Macedonia. Trajkovski was flying to Mostar with members of his government to take part to an economic conference.
The plane departed Skopje at 06:48. At 07:46, ACC Zagreb transferred the plane to APP Mostar. At 07:51, the flight crew reported overhead IAF DIRUK. After IAF DIRUK, the flight crew performed a left turn for 180ยฐ and established heading 146ยฐ, at 4,700 feet. During last 60ยฐ of the turn the flight crew noticed that indications of DME and VOR were lost. The flight crew reported to APP Mostar that they do not have indication of VOR. After 18 seconds co-pilot confirmed to the captain that indicator VOR/DME has been established again and that everything is under control what was conveyed to APP Mostar by the captain.
At distance of 13 NM to VOR/DME MSR, after the approved by the captain, co-pilot disengaged autopilot and had taken the control of the plane. After passing 7 NM to VOR/DME MSR the plane continued with descending on assigned radial 326. And 31 seconds later the plane reached the altitude of 1,810 feet (MDA). The co-pilot then asked for permission from the captain to continue with descent, on which captain did not reply but ordered to co-pilot to increase the power. After that the plane impacted the tree and right after the ground as well.
