It's pretty easy to fall outside of the map in the Eastern Wasteland right at the entrance to Capital Prime. All you need is a fancy car.
After you've disabled the Authority Bridge lasers, drive over the Capital Prime and ram your car into the entrance tunnel. When you climb out, you'll be placed on top of the car, poking through the ceiling of the hole. It's somewhat difficult to move, but if you get positioned just right while jumping at the upper right corner, you can squeeze through a gap in the collision and fall off of the map.
Since there's so little standing room for the player once you've exited the vehicle, the game naturally has to place you where it can find standing room, and unfortunately that standing room pushes you through the ceiling. Being halfway embedded in a collision field bypasses a lot of potential safeguards they might have put in.
I imagine this wasn't found by testers (though I could be wrong) because the visual of the narrow tunnel implies that you have to go on foot, even though it's not explicit or restrictive in any way. You get somewhat programmed into thinking you need to leave your vehicle once you reach a destination on the minimap, so ramming your whole car into it probably wasn't expected.
Luckily, this is a fairly minor issue, even though it results in death for the player. You have to be looking for a gap and looking to exploit this oversight to actually fall victim to it, especially since you can return to your vehicle at any time and simply reverse out of the tunnel.

A special little add-on to the Crawlphin Dive lets you float a bit off the ground while you do it.

Anywhere you have space to run, sprint and perform a normal dive, but in the middle of the animation, press prone (Ctrl) a second time. This should make Amber land higher than she typically should, treating the floor as if it were two feet higher. Additionally, you can still perform the fast crawl by diving, rolling, and then going prone in the middle of the animation, letting you not only float, but fly across the airways.
As an aside, you can use this to easily get into the Floor Assassin state.

As I think Ive demonstrated by this point, the diving mechanic doesnt have as many safeguards as it needs to prevent multiple actions. These multiple actions confuse the game and result in a lot of buggy player states. This can be fixed by locking out other actions during the Dolphine Dive animation, which shouldnt hinder gameplay too much, though I will miss shooting while diving. Whats odd is that the game locks out camera control during a dive, but practically no other forms of input that Ive found.
While I like it, this add-on doesnt provide much that the previous Crawlphin Dive couldnt... beyond floating. Potentially it could offer you a higher vantage point if you needed to dive and then fire, but the added height doesnt seem to affect your interactions with objects for the most part. But as usual, its entertaining. Since the game is still in its early builds, its hard to see practical applications for some of these glitches at the moment.
