The process begins with obtaining approximately 60cc of bone marrow aspirate from the posterior superior iliac spine, which is then processed using the Arthrex Angel® system to create a concentrated solution. This 5cc concentrate is mixed with 5cc of AlloSync™ (demineralized bone matrix) and 1cc of contrast agent for visualization during the procedure.

The preparation involves dividing the mixture into 5 one-cc syringes, which Dr. Saluja prefers for cases involving multiple lesions as it allows for easier allocation of material based on lesion size. After completing the arthroscopic portion of the procedure, including meniscal debridement, fluoroscopic guidance is used to identify the entry point for the injection.

Using the IOBP closed-tip delivery cannula, which Dr. Saluja selects specifically to enable more circumferential filling of the lesion, the mixture is injected parallel to the joint line. The technique includes gradual impaction of the delivery system, rotation of the device to fill anterior and posterior portions of the lesion, and progressive withdrawal of the system while continuing to inject. Fluoroscopic imaging confirms complete filling of the bone marrow lesion from multiple angles.
