Tomb of Horrors wasn't built to punish players in general -- Gary Gygax built it in 1975 to kill two specific characters at his own table, then turned it into the deadliest tournament dungeon in D&D history.

Everyone knows Tomb of Horrors is brutal. Almost nobody knows it started as a grudge match against two named characters in Gygax's own home campaign, then became the official killer dungeon for the first Origins convention.
