It seems that if you item swap a goal tape above the start of the level, the exact frame it spawns, triggering it will give you the normal exit as usual. However, when you return to the overworld, Mario moves upwards instead of the direction he is usually sent. This can be exploited to enter the level located on the unrevealed warp to Special World, where we can use the (?) orb to get another unintended exit. Whether this can be reliably done in real time is still uncertain.

This exit can be done in realtime, but with a modified setup: youtube.com/watch?v=tteioKYydR0

Some more technical details:
* Although it isn't well-known, baby Yoshi sucks in sprites the same way adult Yoshi's tongue does. Thus he can be used for item swaps in some very particular circumstances. In this case, the item swap was done using the sprite overloading method.
* The goal tape must be swapped above the screen before its initialization routine has a chance to run. This means that baby Yoshi must be in a higher sprite slot than the goal tape, and it also means that you have to actually walk right to load the tape, rather than holding R.
* This glitch cannot be done after getting either of the standard exits in Star World 5. If you have the secret one, the star will already be revealed and therefore unenterable. If you have the normal one, beating the level won't cause you to move up.
* Since you get the normal exit from activating this glitch, but move upwards instead of left, it will NEVER be possible to move left from SW5 on your save file.
* The reason goal tapes have strange behaviour when their INIT routine runs above or far enough below the level, is that the "extra bit" that determines what exit they activate is determined by a large offset to the Y position. The reason the tape can be triggered from beneath, is that the tape being above the level actually wraps around as in considered being far below the level.
* In this video, the baby yoshi is in slot 9, and the shell that gets replaced by the tape is in slot 8. However, the only restrictions are that 1) the yoshi is in a higher slot than the shell, and 2) there are no enemies in a higher slot than the shell (other sprites are fine). And of course all sprite slots must be full.

Although this is the 102nd exit to actually be tested, we have reason to believe that six others are possible using previously-known glitches, as seen in this pastebin: pastebin.com/wqk0KfbM. The theoretical maximum is actually 111, but the final three are unlikely to ever be possible, unless someone finds a way to enter an already-revealed star road tile.
