All 21 Codex entries in Chapter 4, in route order, each one shown in the room it sits in.
Codex entries are the quietest collectible in the game. Almost none of them are on the critical path, most are on desks and shelves in rooms the case gives you no reason to enter, and the counter does not tell you which area you are short in.
The chapters are named for the entry, so a single missing page is one click away rather than a full rewatch. The last three all read Departure, which is the game repeating itself rather than a mistake on my part.
