It's unclear whether Ash is dead or alive, but he definitely doesn't need to breathe underwater.
My take on completing the third cartridge of the game, River City Rancid, without dying.
Doing so unlocks "River City Rancid - Unbreakable" achievement.
You can find this cartridge in a secret area, the entrance to which is located at the top left of The Split, leading back to Overflow 20.
