Panic! Dizzy, from the Excellent Dizzy collection developed by Interactive Studios and released on the Game Gear handheld console by Codemasters in 1994 .

"It's puzzling, challenging and mega playable! Play any of the Yolkfolk and challenge friends to a choice of 3 brilliant, original puzzle games. There's shape shifter, dizzy dice and picture puzzler to boggle your mind and blow your brain. Push and shoot the descending shapes in a race against time! Fantastic fun! "Put your brain & reflexes in to top gear for 3 original puzzle games!"

This version of the game contains only three puzzles. Shape Shifter, Dizzy Dice and Picture puzzle. The same game was due to release on the Master System but didn't although there is a partially working prototype rom available.

Picture puzzle is a tile slidding game where you must move tiles into the right place to complete the image in a fixed amount of time. Shape Shifter has you matching shapes 4,5 or 6 on a single horizontal line. Dizzy dice you must match the line in there respective direction. Horizontal, Vertical or Diagonal

In each game you ca choose a level of difficulty which effects the drop rate in shape shifter and dizzy dice and how mixed up the pictures are in picture puzzler. The reward for playing higher difficulties is a higher score multiplier at the end. In picture puzzler difficulty also effects how many pictures you get to play. (Dizzy might be enough for all pictures but I took zak's to be sure)

Speaking of Zaks, he is an unlockable in this version for getting 30,000 total score in any game. Dizzy Dice seems to be the place to do this playing as Dizzy for the 6x multiplier so you only need 5000 points in the stage not that that's a simple task at all. Picture puzzle might be better if you can work with in the time limit.

Overall, this is the game as it meant to be, challenging but doable. The NES version may have added more puzzles but in its current state is almost unplayable as it aims to wipe you out even on the easiest difficulty level with thee drop rate increasing the closer the blocks are to the bottom.
