One of the biggest shifts I made with methane SIBO was moving away from thinking that killing methane is the first priority.

About 70–80% of the methane patients I work with feel significantly better once we get their bowels moving properly and this is almost always before we’ve even started any antimicrobials.

There's one patient (of many many similar cases) who came in with the classic picture od methane SIBO = bloating, constipation, gas, significant distension and very sluggish motility. She had already confirmed methane with breath testing.

As with many in a similar state she had already tried antimicrobial after antimicrobial without getting any sustained improvements.

First step, get her bowels moving and look for obvious improvements. The next question in what improves and what doesn't?

It doesn’t mean the methane doesn’t matter or that antimicrobials aren't useful. They absolutely can help. BUT if we kill the overgrowth without addressing the slow, sluggish gut that helped create the environment for methane to thrive in the first place then relapse is much much higher. I would actually expect it!
