This is a hack for Dr. Mario on Game Boy by marc_max to give full color to the original GB game.
All parts of the game are now colorized, allowing for a much more similar aesthetic to the original NES version of the game, compared to its original version, which used white, black, and shading to differentiate between 3 different colors of viruses.

Then apply this after applying the color hack's patch for some extra polish/fixes if you wish to continue using the 1.0 version of the hack:
