Obtaining all Crystals, Gems and at least Sapphire Relics and beating Cortex again gives you 100% and the true ending. If all the Relics are at least gold then the 45th Gem appears in the middle of the warp room and brings the total to 105%!
After getting 100%, levels also start displaying Platinum Relic times for that one more challenge.

There is actually no story whatsoever other than background flavor and whatever WAD you play just plays the same way with the same intermission screens and the like.
This turns DOOM into a Shadow Warrior or DMC like experience. I decided to play the stock DOOM 2 maps because I first need to learn how to play it. It is not... easy. The game is always set on Ultra Violence but the difficulties influence your Life Points and monster aggressiveness.")
