This is my take on apmc to improve the soundscapes but still keep its rough and harsh nature.

Stage 4: Moments are no longer cohesive due to the patients dementia. Memories are mixed together and broken to short bursts to points of insanity. They have completely gave up trying to remember because they can't think without failing to remember. Their surrounding around them are confusing and a maze to them. Things are only getting worse in later stages
