Oakland Fire Department, founded in 1869 and home to the most catastrophic urban fire in US history in 1991, built MACRO four years ago: a unit trained specifically in de-escalation, created to take mental health calls away from a police or fire response that can turn dangerous quickly.

It's a deliberate choice in a city where 85 percent of calls are medical, not fire. The department can still turn around a five-engine strike team for a wildland fire 30 miles away on a moment's notice, and over half its personnel have five years or less on the job, with leadership built to grow from within.
