Unprocessed Emotions: How They Manifest as Physical Symptoms
What actually happens in the body when emotions go unprocessed? For neurodivergent adults with alexithymia, this is not a philosophical question — it is a lived physical reality. Unprocessed emotions do not simply disappear. They accumulate in the body as physical symptoms: chronic pain, fatigue, gastrointestinal distress, headaches, immune dysfunction, and the persistent sense of a nervous system that cannot settle. This video explores exactly how unprocessed emotions manifest as physical symptoms, what the research tells us about the mechanism behind this process, and what it means for autistic, ADHD, and AuDHD adults trying to understand their own physical and emotional health.
The pathway from unprocessed emotions to physical symptoms is grounded in the neuroscience of interoception and alexithymia. Brewer et al. (2016), published in Royal Society Open Science, proposed that alexithymia represents a general failure of interoception — not just emotional interoception, but the entire capacity to read internal bodily signals including hunger, temperature, pain, and physical sensation. When emotional arousal cannot be consciously identified, it persists as unresolved physiological stress. Over time, that unresolved stress contributes directly to physical symptoms. A 2019 meta-analysis of 77 studies involving over 8,000 people with chronic pain found significantly higher alexithymia in chronic pain populations than in non-clinical controls, with a large effect size and difficulty identifying feelings as the most strongly associated alexithymia dimension (Aaron et al., 2019, Pain).
