Alright, this is pretty much what the Survey Box does. This is an exclusive for the Wiiware game, WarioWare DIY Showcase. There are no survey options in the DS version.
When you send a game, Record, or Comic to your friends through the Wii, they have the option to fill out a survey for whatever you sent them. In this video, I show what a couple of surveys look like when they've been sent to me. They're shipped into this red box in the main menu. When you have new surveys sent from your friends, there's an icon at the top corner notifying you that you have new surveys in.
The surveys consists of ratings (1 to 5 star ratings), and limited comments. When I say limited, you only have a number of selected comments that you can apply for the survey you fill out for other people's creations. I've also filled out a survey for a couple of ppl on my FC list. Maybe I'll make a video of how I do the surveys of my friends creations next time.
