Armor, Stealth, and Charisma are impossible to maximize together in a single outfit in Kingdom Come: Deliverance II, but there are several best in slot items that can help you get close.
I chose to prioritize high armor rating while balancing visibility/noise/conspicuousness.
Depending on your stats, perks, and use of alcohol and/or perfume, there may be many different ways to achieve 30 charisma, or at least to pass very hard (25+) charisma checks.
Using the "Well fitted" craftsmanship perk, you can maintain 0 noise by using the armor pieces shown in this video, along with a shield (-5 noise). There are better stealth armor pieces, but they have a massive drop in defense, so are not worth using in my opinion.
