Finally, at long last, it's done. Sub 3:20 in this category is something I've wanted for a very long time, and I finally got it - on a random run in a Pacifist session no less! Very happy with this, a few mistakes here and there but generally very well played :)

Since this run, redrobot found you could do low% with one item
Low% Definition: Beat the game with the fewest amount of upgrades which includes heart pieces, energy pieces, and anything in the UI (tier two upgrades are counted twice). The current known lowest is 2 upgrades, being Potion 1 and Glove 1.
(as of now) an unofficial category, the routing is very different from the regular gold category! this run is probably the most difficult to get right compared to gold or maybe even cherry, the tricks used are pretty tough to get first try. the ending gearface section is also awful, even though sub 12 is definitely possible with this route, im not sure i want to go for it")
