In this detailed surgical walkthrough, we demonstrate exactly how to perform a Small Rotator Cuff Repair using two double‑loaded Knotless 2.6 FiberTak® Soft Anchors.
This technique is ideal for small, laterally based cuff tears and offers strong fixation with minimal bone removal. The surgeon begins by establishing visualization using the Pano™ scope, allowing an ultra‑wide panoramic view of the entire greater tuberosity and the full extent of the tear. With the push of a button, the surgeon alternates between a 70‑degree and standard 30‑degree view to assess tissue quality and plan anchor placement.

Once the tear pattern is confirmed, an ASL portal is created for dedicated suture management. Two anchors are planned: one anterior and one posterior, both positioned precisely along the lateral edge of the greater tuberosity. The working sutures from the first anchor are retrieved, and using the scorpion suture passer, the surgeon performs the first tissue bite through the anterior portion of the tear. After retrieving the stitch, the same process is repeated to capture the second portion of the cuff, ensuring both bites are on the correct side of the previous sutures to avoid tangling.

Attention then shifts to the posterior anchor. After confirming optimal positioning off the tuberosity edge, the black‑and‑blue working sutures are retrieved. Again, the scorpion is used to carefully pass sutures while keeping track of spacing between previous stitches. The wide Pano™ scope view aids in confirming suture spread and preventing overlap. Once both sutures are passed, the conversion loop (shuttle loop) is used to feed the repair sutures through the anchor, maintaining correct orientation by applying gentle counter‑tension.

One of the key pearls in this technique is ensuring that the suture conversion loop is used to avoid twisting and to keep the sutures perfectly aligned as they enter the anchor. This smooth passage preserves suture integrity and improves final reduction. With the repair sutures converted and tensioned, the surgeon uses the MegaLoader cutter to seat each suture down to bone, securing the cuff tissue firmly onto the prepared tuberosity footprint.

The final construct results in a single‑row lateral‑row repair with excellent tissue compression and footprint restoration. This method is efficient, replicable, and ideal for small tears requiring strong fixation with minimal hardware. Surgeons can adapt this approach to similar tear patterns and benefit from the streamlined suture management facilitated by dedicated portals and clear visualization tools.
