To preform this glitch, the player needs to have a save file where the Itchy & Scratchy ticket has not yet been purchased in Level 3 (located in the Android's Dungeon). Then, begin any mission and fail it while inside the Android's Dungeon. Once you fail, either choose the "NO" option when asked to retry or instead choose "SKIP". The game will crash shortly after the option is selected.
This seems to happen because Radical hacked in the Itchy & Scratchy ticket pretty badly, and the game seems to have trouble re-enabling it if you're right beside it.
This footage was recorded not only as an excuse to test my Diamond VC500 capture device, but also to demonstrate how the console releases handle game-crashes. It's slightly more amusing than how the PC release handles it, with the music looping the part right before it crashed over and over.
Note that this glitch must be preformed in a save file where the Itchy & Scratchy ticket has not yet been purchased. If the player has already obtained this from the Comic Book Guy, then the game will cancel the mission properly.
