Patrick Stone on the team also shares some insights as to why PSUs might fail, which parts commonly fail, and opens up the NZXT PSU to inspect. We don't think this is some widespread issue with NZXT PSUs -- most likely just an unlucky event because of the odd use case. Still shouldn't happen, of course.

We had these boards in testing for nearly half a year, but ultimately tore it all down so we could reclaim the space. We spent about $1400-$1500 on motherboards, a lot of money on staff time, and ultimately, the ASUS Z690 Hero motherboards simply didn't die from the flipped cap issue. That doesn't mean they WON'T die, or that it's not a problem, but we simply couldn't recreate it. That's the challenge with defects that don't trigger 100% of the time on a reliable event.
