The carpal tunnel syndrome is one of the most common peripheral nerve entrapments. Though it can present with predominant sensory symptoms, chronic cases can cause motor involvement. The "pen test" is performed to assess the integrity of the Abductor Pollicis Brevis muscle (APB) which is supplied by the Median nerve after it traverses the carpal tunnel.
