The old Account Monetization Program has been renamed and merged into the Partner Program. Old perks such as banners and custom thumbnails are no longer something you apply for, but rather I suspect this is something you unlock by building and growing your channel.
The old Partner Application page youtube.com/Partners now redirects here youtube.com/yt/creators/partner.html which is the start up for monetizing videos. Once you start monetizing you are now considered a Partner.

You are also allowed only one AdSense account per the Terms of Service but if you have multiple accounts they can all be linked to the same AdSense account.
