This is an early beta version of Major Stryker, an arcade shoot-'em-up released by Apogee Software in 1993.
There was a legendary beta version - the very first version - that was so hard that all the beta testers complained. In response, Apogee dialed down the difficulty a bit, and that's the version in this video. Apogee released it on January 16, 2018, one day after the 25th anniversary of the finished version.
