I've encountered a glitched out faceless uruk captain, named Norûk the Whiner. He is a poisonous outlaw tank.
This rare orc was found in the realm of DiamondsHands. So, today, we avenge DiamnodsHands who died at the hands of Narûk the Whiner. Naruk has a total of 12 kills and he doesn't even have a face.
Glitched out HEADLESS ORC, super rare and epic.
Norûk is almost a cursed name given the pathetic orcs that hold this name.

What is up with Eli Vance? Do you notice it? Can you see it? What is happening with Eli Vances eyes ? Is he a cyborg? Did the combine do something to him?
Both Alyx Vance and her father Eli Vance may have joined the combine and while they try to mask it and present Gordon Freeman with a friendly face, clearly, theyre showing signs of their true inner selfs as everyone can see within their eyes! This could be the plot twist that was to be revealed in half life 3. Is this the ending that you wanted ?
I am playing the half life 2 remaster when I encountered this.
In all honesty, I think there is something going on graphically but no idea what the details are. I am playing the recently remastered version of half life 2 which supports high resolution screen displays (awesome!).
