Normally, heart pieces work like this: The default value is 0 and it increases by 16 for every heart piece you get. When this value is between 64 and 79 inclusive, the value will subtract 64 and a heart container is added to your health when you cancel a text box. However, voiding out without canceling the text box will not add the heart until you cancel any text box.
This trick manipulates how the heart pieces work to delay the health addition for either until you cancel a text box or you get enough heart pieces to wrap the value around from 255 back to 0.
Using Down A collection delay, I fell out of bounds as I collected a 4th Heart Piece. This caused my Heart Piece value to increase by 16 from 48 to 64, but did not add a heart because I never cancelled the text box. This has some interesting side effects. The first time you cancel a text box, you will immediately have the extra heart added (and the value will go to 0). I talked to the man to do this and gain the 4th heart.
However, if you collect another heart piece before canceling any text box, your Heart Piece value will continue to increase by 16 (past 63) and you will not be able to gain more heart containers from heart pieces because it will now be 80 or above. This means you can increase the value all the way up to 255, from which it wraps around to 0 again.
A very interesting effect happens when you get a heart piece with values 64-255. You will get the wrong text, which can be seen here:
