This happened here in Chapter 18 (Gears of Apollo puzzle) in early August 2013 while I'm going for the Platinum trophy.

I'm no expert but I usually play at a high level pace this game. Not only did the camera forgot to zoom out, notice there are no fight scenes against the Empusa as she never showed up in this vid (she was supposed to appear as soon as Kratos landed in this area, THEN the Manticore spawns would begin to hatch)

By the way this glitch happened hours AFTER the latest GoWA patch
Continue to fail Todd Papy
