In Forgive Me Father, the Journalist awakens in the nightmare-ravaged town of Pestisville, a New England settlement consumed by a mysterious plague. The dead have risen as horrific creatures, while fanatical cultists worship dark Lovecraftian powers. Determined to uncover the truth, the Journalist travels through the town, abandoned buildings, mines, churches, swamps, and ancient ruins while collecting documents and photographs that reveal what happened to Pestisville.
As her journey continues, the Journalist becomes increasingly consumed by madness. The horrors she encounters become stranger and more supernatural, eventually leading her toward an ancient evil connected to Cthulhu. She fights through hordes of monsters and cultists before descending into the Abyss, where she confronts the source of the nightmare.
The Journalist ultimately battles and defeats Cthulhu, seemingly saving Pestisville from destruction. However, the victory comes with a horrifying revelation: the events of the game were not happening in reality. The Journalist is actually a patient in a mental hospital, and the entire journey through Pestisville was a hallucination brought on by her deteriorating mental state.
The final revelation suggests that the monsters, cultists, Cthulhu, and even the destruction of Pestisville were manifestations of the Journalist's disturbed mind. The horrific adventure was her perception of what was actually happening around her while she was undergoing treatment. Story details scattered throughout the game—including patient records and other documents—provide clues to this interpretation.
In short: the Journalist believes she is fighting through a Lovecraftian apocalypse and defeating Cthulhu, but the truth is far darker—she is a psychiatric patient whose mind has transformed her treatment and surroundings into a horrifying supernatural adventure.
