It's Graphic Simulations' F/A-18 Korea, a follow-up to F/A-18 3.0. This is basically more of the same but with a hardware graphics overhaul. It was a top of the line simulation with a flight model and physics that was the gold standard, up until Falcon 4.0 some years later. Only A-10 attack/A-10 Cuba could compare with just how darned smooth the physics are in this game.

The game itself earned some praise for it's graphical effects that ran well on even a mid-range voodoo equipped PC. IE: being able to hit the game's hard-coded max frame rate of a blistering 20 FPS! (with the option of kicking it down to 10 if your PC needed to).

The newest release of Dosbox Pure (Release 6) fixes up some issues with hardware 3DFX emulation. Notably, the 2-d elements in-game now display properly. That's great because this game benefits quite a bit from DOSBox Pure being able to up-rez the 3DFX render from it's humble 800x600.
