Commodore 1571 floppy disk drive S/N JA4 015842 repair. Bad 6502-A CPU, probably failed A15 line driver or internal connection. Identified with both oscilloscope and logic probe. In my repair experience, bad 6502s (not abused) are rather rare findings.
Also a few informations on how to use 1571's double side format without a Commodore 128 computer.
