Anybody with a CNC mill or even a manual mill and some decent skills can make one of these, but if you want to spend your time machining your own projects and ideas instead of a simple mill vise handle you can buy one here: cncmolds.com/webstore/speed-handle-for-mill-vise.html

I wasn't going to sell them. I think anybody who has a need for one can probably make one, but my buddy convinced me there are guys who would buy one so I set up for the most common sizes and added them to my website.
