The boss itself is pretty straight forward but has multiple skills that hit very hard and all of them cleave at least 180 degrees in front of her. One attack in particular is essentially an instant death if you don’t have over 1000 health and good P.DEF and Thrust Def. When fighting it you’ll want to do a hit and run style by baiting attacks and then countering. Do not under any circumstance let it freely do long range abilities and do not try and kill it from behind.
Every attack that the nurse does should be dodged by going backwards the majority of the time. If you attempt to go to the side you will be hit and going behind her will usually result in a hit, unless you time it absolutely perfectly. Backwards equals safety.
The main abilities you want to look for and counter are when it crosses its arms to do a long windup cleave, a triple wide arcing cleave, and a rapid six armed assault it does while walking forward. During the rapid slashing attack the nurse will attack in a 270 degree radius, leaving only directly behind it open. It also walks quite fast so using a long ranged or quick attack is your best bet for following up on attacks.
If it lifts all of its arms above its head you need to get backwards very far and fast, or dash behind it. That is the attack that can one shot, easily. Occasionally it will do a 180 spinning attack but it is rare and only if you are consistently behind or on the side of it, which you shouldn’t be.
The last thing you absolutely need to look out for is when it stops and starts to channel a purple cast. Do not under any circumstance let the nurse do this, because it will create a huge purple mist effect in the room. During the mist it will also have a mirror image clone that randomly attacks, all the while teleporting around non-stop. Just rush to the boss and unload on it whenever you see this cast because it is free damage and it will quickly be interrupted.
If for some reason you are kiting the boss around or staying too far away it will also lunge. It is easy to spot due to it lowering all of its blades while leaning forward and hovering. Just dodge straight forward when it lifts off the ground and spin around to get off free hits.

In order to make someone kill one of their allies you must first learn Possession (Level 2), which will cost a total of 8 runes, as well as Bend Time (Level 2), which will cost you 10 runes. Due to their high rune costs you will not be able to learn them both until Mission 3: House of Pleasure at the earliest—that is if you find every rune up until that point.
The most reliable method is to have a soldier shoot one of their allies with a pistol, and is the same method shown in the Harms Way guide. Mission 9: The Light at the End is especially good for doing this—which is the location shown in the video—because every soldier has a pistol. The first step is to locate two more soldiers in the mission.
About two seconds after an enemy points their pistol at you they will fire. It is the same delay for each and every type of enemy. The cast time on Bend Time is also about one second. For a kill to count, simply use Bend Time (level 2) a second after one of them fully extends their pistol and aims it at you, or reloads after firing. The moment bend time activates the target should have just fired their gun.
Immediately use possession (level 2) on the soldier that did NOT shoot the pistol; if you possess the soldier that fired it will count as a suicide when they die to the bullet. Walk the possessed soldier in front of the bullet (be sure not to walk into it), and position him a few feet away from it—not right in front of it otherwise it will miss—then release the possession. It will instantly kill them and count towards the trophy. If only one soldier remains immediately flee and find an additional one.
It is best to have at least five or six mana and health potions before attempting this and to save first. You should also save right after you perform a successful kill. By far the easiest method of unlocking this trophy/achievement is by combining the Shock Jockey and Undertow vigors on regular, non-Heavy Hitter or armored enemies. You will not be able to learn Undertow until Chapter 28, so dont worry about performing this before then.
Once you have learned it simply equip them both simultaneously and go find some weak enemies. Hit them with a Shock Jockey first then immediately follow it up with an Undertow. They will die from the electrocution while in mid-air. If you are playing on either Hard or 1999 difficulty, simply lower their health to 50% or so before doing the combo.

This is one of the 50 quests that you must complete in order to unlock all of the treasure art in the game. Read below for a full walkthrough of how to complete it.
Quest: Submerged Memories - At Lost Woods, steer a small boat into the vortex.
Art: Scylla in the Depths
Prerequisites: Beat the game and turn in any quest, or finish the Hermit quest chain in the Lost Woods for the second time.
Area: Lost Woods (Route A)
Walkthrough: At the very end of the stage you will ride begin riding a small boat. While riding it you are typically supposed to avoid the swirling vortexes and oncoming fish, but this time you want to drive directly into them and let the fish land in your boat. Once your boat sinks you will have completed the quest.
Note: Because you need to intentionally sink the boat it is best to do this solo, rather than in online co-op. People will stop the fish from jumping into the boat and forcibly steer you away from the vortexes. This will result in you having to repeat the stage for the quest.

Gunzerker is the class action skill for Salvador, the Gunzerker. You unlock the skill at level 5, but to actually gunzerk for additional time you must reach level 21 and learn the skill Yipeee Ki Yay—which increases the duration of Gunzerker for every enemy killed. If you are already level 23 or higher just skip the next paragraph.
At level 21 with one skill point in Yipee Ki Yay, every kill will increase the Gunzerker duration by 0.6 seconds. However, if you use a class mod that has +skill to Yipee Ki Yay you do not need to go beyond level 21 to unlock the trophy. So if you find a class mod that has the +skill while leveling, save it. If you do not find one before level 21 just go to Sanctuary and look in the Medical vending machines at Zeds office and in front of Marcus gun store until one is for sale.
Once you have the class mod you can head down to Marcus store and begin the mission Rock, Paper, Genocide: Fire Weapons!.If you have already completed thiis mission you will have to join the game of someone that hasnt done it yet. It becomes available the moment you reach Sanctuary for the first time, so its pretty easy to hop in a game with someone that needs to do it still.
The mission requires you to kill some enemies with fire elemental weapons: do not use a fire elemental weapon otherwise he wont respawn. Use any type of weapon that isnt fire tyoe.
Ideally you should equip your most powerful *non-rocket launcher weapons* (they do not regenerate ammo), then gunzerk in front of the enemy. Aim for the head and kill him as fast as possible. When your reticle turns red start firing; he can be shot before he becomes visible from the shadow area.
It may take a few attempts to get right, but its the easiest method in the entire game.

This is the secret/true ending to the game and one of three possible outcomes. It is also quite easy to miss if you do not follow the right steps in the correct order, kill the wrong enemies, and/or mishandle some items. I suggest saving this for last since it is the hardest ending of the three to complete as well as the “true” one.
To get this ending you will need to collect at least three of a special item called “One Third of Umbilical Cord”. Despite the name there are actually four separate ones that can be found throughout the game, which is actually good incase you make a huge mistake somewhere along the lines regarding one of the four. Fortunately two of them are impossible to miss, so as long as you get the final one you are good to go.
Here are the four locations of the cords, in the order that you can find them throughout the story progression:
Cord #1: ON THE MEMORY ALTAR IN THE ABANDONED WORKSHOP
Cord #2: KILLING IOSEFKA IN HER CLINIC (Missable)

[You brought about the Lord Regents fall from grace by broadcasting his crimes]
During Mission 6: Return to the Tower, you will travel back to Dunwell Tower to kill Lord Regent. Upon arriving there Samuel will inform you about the broadcast tower that all of the propaganda is aired on, opening up an optional mission to travel to it and investigate.
After making your way through the heavily guarded yard you will finally reach the mansion. If you entered through frontmost method you will find yourself in the main lobby, as shown in the video.Once you do use Blink to ascend the chandeliers and sneak into the Lord Regents chamber. You will see a safe to your right; the combination is 935. Open it and take out Lord Regent confession audiograph.
Exit through balcony and Blink towards the left hallway. Turn right and travel down the hallway until you see the Arc Pylon. If you have Bend Time 2 use it to walk past easily, otherwise use Blink to travel just above it onto the stairs. Walk up the stairs and activate the broadcast system to unlock the trophy/achievement.
This is also the fourth and final target you must neutralize through indirect means for the Poetic Justice trophy/achievement. However, it will not unlock until after you have returned to Samuel and completed the mission.
