Blocked bile flow is one of the more common reasons I see SIBO not properly resolve, especially in people who have already done treatment and felt better for a period of time and then slowly saw their symptoms return.
Bile isn’t just there to digest fats. It plays a major antimicrobial role in the small intestine, helping keep bacterial growth controlled while also supporting proper motility through the gut.
When bile flow is sluggish, thick, or not being released properly motility can slow (think constipation, bloating, food just sitting there) and bugs can start to build up - think SIBO/SIFO.
SIBO treatment reduces the bacterial load, symptoms improve, but the underlying driver, poor bile flow, hasn’t changed. Over time things drift back to the original set point - aka SIBO regression!
