On August 22, 1992, an Atlas I with a Centaur upper stage launched from Cape Canaveral's Launch Pad 36B. The payload was the Hughes Communications Galaxy 1R communications satellite. The mission failed due to an engine valve problem with the Centaur upper stage.

Although the title card says the booster was an Atlas II, multiple sources confirm it was an Atlas I.

A UPI news report about the launch failure is at upi.com/Archives/1992/08/22/Atlas-1-rocket-fails-on-launch-multi-million-dollar-satellite-destroyed/6622714456000/.
