This whole lower river area to the far southwest of Crumbling Farum Azula is filled with nasty large and one gargantuan Wormface - corrupted, bile spewing abominations that cause rapid blight build up. Ranged attacks help, though my build didn't really have much of that.
Died to them enough I didn't bother fighting them after later run backs and just tried to get to the big guy. Really tough. Stand in front during one of his moves and he'll grab and devour you, compiling to a one-shot as you're eaten. Sadly, he doesn't even drop anything worthwhile but neat I could get it down with spirits.
