A few years back, I injured my knee. Dr. Fleischlii had to take out my cartilage because of a joint injury. I managed to rehab it back a bit, but I injured again a year ago and all I want to do is compete as a runner again. I was a bit disheartened with the news I got at the doctor. But why can't I run like I used to? What can be done. Here are some options:

Slalom Kayaking and Canoeing is an olympic sport that involves getting down a whitewater course faster than your competition. The courses are full of red and green gates. Red gates are "up" gates and green gates are "down" gates. There are from 18-25 of these on a course. Your head must go through every gate. If you touch a gate there is a 2 second penalty. If you miss a gate there is a 50 second penalty. The full rules are obviously a tad more complicated but we wanted to give the basics here - especially for all your slalom kayakers who want to share the rules of the sport with your friends.

Very simply ligaments are around to hold our bones together. It’s all part of this thing we like to call the musculoskeletal system. People sometimes confuse tendon’s with ligaments. While they have some similarities, tendons attach muscle to bone and ligaments attach bone to bone.
If you look at any joint, you’ll find a whole array of ligaments functioning to stabilize and limit the movement of the bones within (showing xrays of different joints on a real person). Take the knee for instance.
You have the Femur here and then the tibia and fibia. On top of that you have your kneecap or patella. Remember Ligaments connect bone to bone and in the knee lets show these 4 major stabilizing ligaments. This is your ACL. Here is your PCL. Here is the MCL and here is the LCL.
Now if you’ve ever tweaked your knee ligaments, or any other ligament in your body - say spraining an ankle, you’ll probably want to know what you can do to strengthen and heal them.
Unfortunately for us, torn ligaments are not like broken bones. They won’t just reattach themselves.
Here are the basics:
Ligaments are composed mostly of long, stringy collagen fibers that create bands of tough, fibrous connective tissue. Ligaments themselves are slightly elastic, so they can be stretched and gradually lengthen, increasing flexibility. But if stretched beyond a certain point, ligaments can become overstretched and compromise the integrity of the joint they are supposed to be stabilizing — so stretch with caution.
As a little tidbit, the term double-jointed actually refers to people who have highly elastic ligaments, which allow them to move their joints into more extreme positions than most people.
If you did injure a ligament - go to the doctor. Trained orthopedic docs are usually the best at diagnosing these injuries. Work with them and the physical therapy staff to find a rehab routine that suits your needs. There is no denying that cortisone reduces pain and swelling. Because of that a lot of cortisone shots have been given to help people healing from joint injuries. But just because you get immediate pain relief doesn’t mean it’s helps your joint heal.
A recent analysis of studies on thousands of individuals with joint pain, particularly tennis elbow, showed that cortisone shots, did in fact reduce pain immediately. However, when they examined individuals 6 months later, they found that those with cortisone shots had a lower rate of full recovery and were 67% more likely to have a relapse of the injury. How is that possible?
To understand it you have to know more about cortisone.
Cortisone is a corticosteroid. Its produced naturally in the body via the adrenal cortex (kidneys). If you’re familiar with cortisol aka the stress hormone, it works similarly.
You see, when your body is in a stressful situation, this hormone is released. It works like this, you want to get sugar to your brain - the most important thing. So the hormone, stimulates the breakdown of fat, protein and carbs into useable energy int eh blood. That means your blood glucose levels rise. It also suppresses other activities that aren’t vital in that immediate stressful situation, like the immune system. And that’s the main role of a cortisone shot - it’s as an immune suppressant aka inflammatory.
So, you can use it medically to cure problems with immune response - like
The problem is that the they are best used as a short term solution.
Think about it, you don’t want your body filled with stress hormones long term because, there are side effects to what we associate with having high stress levels. Namely:
mood swings and sleep disturbances hypoglycemia decrease in the bodies natural corticosteroid production decreased bone density fat atrophy.
Fat atrophy probably sounds good, except when it occurs at the point of a cortisone injection.
In the end, there are somethings you would not use cortisone for - like rotator cuff disease Shoulder pain - like adhesive capsulitis spinal injuries like cervical radiculopathy knee osteoarthritis
Of course, with that said, the science does point to cortisone helping with things like: lumber radiculopathy, plantar fasciatis, and morton’s neuroma.
So there you have it, Cortisone, is not the cure all treatment it was once thought to be. Know though that while it helps with some things, it can help with temporary pain relief - and well, it might decrease long term recovery. So if you’re an athlete or someone with a random tendon issue, you might want to think twice about getting cortisone. But, that’s what the doctors are for. This week we went to OrthoCarolina's Shoulder and Elbow Center on Randolph Road in Charlotte, NC to meet with Dr. Nady Hamid. I must say, I learned a lot about cortisone. I came into the meeting with only a bit of understanding, having just had one myself. I knew it was a steroid and I knew it seemed to help. However, I had read some conflicting news on the drug. Dr. Hamid cleared it all up for me.

The unicycle is a pretty simple device: a seat, some pedals, a crank, and a wheel. You wouldn't really think that the idea of gears plays a role in unicycles. In reality the combination of the wheel size and crank size creates a gear. In unicycling they call this concept the total gear ratio. We wanted to discuss this concept, and introduce you to how different wheel sizes actually change what you're able to do on a unicycle.
