A semi-pug formed on the PTR to kill Hagara, one of the new bosses to be released with Dragon Soul. At the end, she bugged out for us and just instantly respawned at full health.
It took about an hour or so of pulls to get it down; and her lightning phase got hotfixed midway through the allotted time frame (before that, the lightning would keep chaining even after the lightning phase was over).
