$Optimizing Post Discharge Care for HFpEF HFmrEF: Strategies to Reduce Readmissions This session explores the unique post-discharge needs of patients with heart failure with preserved ejection fraction and heart failure with mildly reduced ejection fraction and the factors that place them at high risk for early readmission.
