You can buy the respec potion in Velen from Keira, a sorceress encountered during the main quest. Just follow the road above the town Midcopse to her house. After you complete certain quests Keira will no longer be there, so you will have to buy the potion somewhere else. There is also a merchant near the Barber in Novigrad. This merchant is inside a building and it's the little coinbag icon on your minimap. They both sell you the Potion of Clearance for 1,000 Crowns.
