If you are inside a dungeon and the boss is still alive, then you cannot fast travel from the inside of that dungeon. You either need to kill the boss, or leave the dungeon.
Additionally, if you get teleported by a trap chest, such as the one in Dragon-Burnt Ruins which takes you to Sellia Crystal Tunnel, then you need to REST at a site of grace before you can fast travel. This effect will persist even if you leave that dungeon, until you rest at a site of grace.
