Complete the final mission in the QR Code Investigation.

There are 16 QR Codes to find in the QR Code Investigation, the locations can be revealed on the map after you unlock every ctOS tower. You then need to go to these locations and scan the Codes. The codes may be split apart on different buildings and only line up to be scanned when viewed from certain cameras.

Once you have scanned all of the QR Codes you will unlock the final mission and then need to complete the final mission to catch the hacker and unlock the achievement.
