Believe it or not a simple pivot table can do this. No VBA. No Formulas. No SQL cartesian product.

Steps
1) (0:33) Convert data into a table (Highlight data: 'Insert' 'Table')
2) (1:05) Pivot: 'Insert' 'Pivot Table' and then drop all fields into row label area
3) (1:32) Pivot: 'Show in Tabular Form' & 'Repeat All Item Labels'
4) (2:00) Pivot: Turn off 'Subtotals' and 'Grand Totals'
5) (2:13) Pivot: Right click in Pivot, 'Field Settings', 'Show Items with no data'
6) (2:57) Pivot: Filter out blanks
7) (3:27) Double check results
