Build the farm (or anything else that uses the villager AI), not near a village. Build it at least 64 blocks in radius away from the center of a village. These only counts for a "normal" sized village, to be safer build it at least 100 blocks away from the last door of a village.
The villager in the middle must have an empty inventory, so no food in his inventory. Otherwise the farmer don't throw food to the villager. Please note that a villager keeps its inventory when he grows into an adult. To make sure that you have a villager without food in your inventory you can cure a zombie villager, or build my Villager Breeder for Minecraft 1.8.

Don't give the villagers beetroots. They can pick it up, but they can't use it for breeding (1.9.2)!
If the villagers don't pick up the food, please check if the gamerule "mobGriefing" is on false. It must be on true: /gamerule mobGriefing true
Because of questions in the comments:
*Can I have more than 8 villagers in a cell?*
It is much more efficient and produces less lag when separate.You could build a breeding cell with twice the amount (16 villagers) and losing almost no efficiency, but more villagers would have almost no effect. But I would not build the cell with 16 villagers. It lags more. I mean it is not that much more with 16 villagers, but then the villagers also do not form a "circle", they only look in one direction ;-P
*Does the old breeder no longer works in 1.9, why is this breeder better?*
The old breeder works only partially in 1.9, the food flies out and the baby villagers can pick up the food. The villager breeder in 1.8 is actually a bad design. Having all villagers in a holding chamber causes lag and with water it get worse. The water is needed to feed all villagers and to speed up the breeding when they share the same space, but the breeding is still about half efficient as if they are separated. It was a compromise for 1.8 because there was no other way to build it reliable, compact and so that the babies can’t pick up the food.
In the new design for 1.9 the villagers still "overlap", but it causes only about half the lag of what mobs in a holding cell make. And it is twice as efficient, meaning you need only the half amount of “breeders” to get the same amount of villager babies. So it causes much less lag to get the same result and there is no out flying food anymore.
Because it creates a little lag, I would not build it super big. So if you need 500 villagers per hour I would take Ray's Works design: youtu.be/IUqHcn0vw0w

The reason why I implement the "AFK Ender Pearl Farm" is because Enderman rarely spawning on the hoppers inside the base when you arrive (It has to be dark there for the light funnel). I found that way cooler than just to put a redstone lamp there.
For the landing platforms you don't need glowstone / fences you can use torches / slabs.
In the base it is better to use signs instead of the pressure plates, because if ender pearls fall on the plates the red stone signal blocks the hoppers. But that is very rarely and the pressure plates give a better look.

The idea to block bookshelves with water have been around for a long time. There are some videos out there based on that. The video from which I was inspired, I saw a long time ago:
This machine produces around 3000 ice blocks in about 2.5 hours. Depending on how much and where the ice will spawn. I spent about a month to research, build and fail, until I had the idea with the piston worms and it took me over a month to build the ice machine. But in the end it became more and more about a fun project that is only worth to build if you like big machines.
If you want a resource friendlier and easier to build ice farm, take the design from JL2579: http://youtu.be/ew4qopu2zNM It is still the best ice farm out there. If you have big ice projects I recommend to double the size. Or build two of them, so if you harvest one, the other can freeze.
