Clinicians and educators know that time constraints, workload pressures, and hierarchical structures can all impede constructive feedback and productive exchanges with trainees. In this Grand Rounds, medical education experts Brian Schwartz, Bridget O’Brien, Griffin Plattner, and Jackeline Vajta will explore the barriers to productive feedback within the DOM, followed by a panel discussion offering trainee and faculty perspectives on improving the culture of feedback at UCSF and beyond.
Speakers
Brian Schwartz, MD, professor of medicine, is an infectious disease specialist, with a focus on infections in immunocompromised patients. He serves as the Associate Chair for Education for the Department of Medicine. He previously served as the Program Director of UCSF’s Infectious Diseases Fellowship and Course Director for the Pathogens and Host Defense block in the School of Medicine. Brian was the inaugural Chair of the Medical Education Community of Practice for the Infectious Diseases Society of America and the Chair of the Microbiology/Immunology Item Writing Committee for the National Board of Medical Examiners.
Bridget O’Brien, PhD, is a professor of medicine and an education scientist in the Office of Medical Education's Center for Faculty Educators. As co-director of the Teaching Scholars Program and the UCSF-University of Utrecht Health Professions Education doctoral program, she teaches and mentors faculty and learners interested in education research and scholarship. She also directs the Advanced Fellowship in Health Professions Education Evaluation and Research based at the SFVA. In 2015, she was selected as one of five national Macy Faculty Scholars supported by the Josiah Macy Jr. Foundation and in 2021, Bridget was selected as a KIPRIME Fellow at the Karolinska Instituet.
Griffin Plattner, MD, received his bachelor’s degrees in business management and chemical engineering from the University of Florida. He then earned his medical degree at Washington University in St. Louis before completing a primary care medicine residency at Massachusetts General Hospital. Griffin is now a National Clinical Scholars Fellow at UCSF with research interests in medical education, community-based healthcare delivery, and health equity.
Jackeline Vajta, MD, is a fellow in Academic Hospital Medicine. She completed her medicine residency at Saint Mary’s Hospital-Yale School of Medicine. She is passionate about research in medical education and cardiovascular medicine. This year, she received The Academy of Medical Educators Cooke Award for the Scholarship of Teaching and Learning.

In this Grand Rounds, we will hear from a cardiac surgeon, interventional cardiologist, and non-invasive cardiologist about their multidisciplinary approach to complex valvular disease. We’ll also learn about current guidelines for the management of mitral, aortic, and tricuspid valvular disease.
Speakers Tobias Deuse, MD, a cardiothoracic surgeon, is director of minimally invasive cardiac surgery at UCSF. He is well known for developing minimally invasive techniques for mitral valve repair, as well as innovative approaches to heart failure surgery (most notably minimally invasive techniques for implanting ventricular assist systems and mitral valve repair utilizing fully robot-assisted endoscopic surgery). Deuse’s honors include election to the board of directors of the International Society for Heart and Lung Transplantation.
Sammy Elmariah, MD, MPH, is an interventional cardiologist specializing in caring for patients with valve disease, coronary artery disease, and adult congenital heart disease. He is a leader in transcatheter heart valve procedures, including transcatheter aortic valve replacement (TAVR), edge-to-edge mitral valve repair (MitraClip and TEER), mitral valve replacement (TMVR), and new approaches for tricuspid valve disease. His research focuses on the prediction and optimization of clinical outcomes of patients with aortic stenosis and after TAVR.
Kirsten Tolstrup, MD, is a non-invasive cardiologist with a special interest in echocardiography, valvular disease, and structural heart disease. She works closely with the interventional team on transcatheter-based procedures such as TAVR and MitraClip and on optimizing our multidisciplinary team approach to cardiovascular care at UCSF.

While many parts of medical education have changed rapidly over the past few years, other aspects seem remarkably static. This year's Holly Smith lecturer, Dr. Holly J. Humphrey, is an international leader in medical education who is currently President and CEO of the Josiah Macy Jr. Foundation. Her work has influenced the national dialogue in medical education in areas such as diversity and inclusion, mentoring, and professional identity formation. Dr. Humphrey will offer her views on the current status of medical education and how it needs to evolve to meet trainees’ and society’s needs.
The annual Holly Smith Visiting Professorship is the Department of Medicine’s most prestigious yearly lectureship. It is named for Lloyd Hollingsworth “Holly” Smith Jr., MD, a visionary physician-scientist whose uncompromising quest for excellence helped transform UC San Francisco into the world-renowned institution it is today. Smith, who passed away in 2018, served 35 years as chair of the Department of Medicine and associate dean at the UCSF School of Medicine.
Speaker Holly J. Humphrey, MD, MACP, is president of the Josiah Macy Jr. Foundation, the only U.S. foundation whose focus is health professions education. Prior to her current role, she served as Dean for Medical Education and as Internal Medicine Residency Program Director at The University of Chicago. She was selected as a favorite faculty teacher by graduating University of Chicago medical students more than 25 times. A pulmonary and critical care physician by training, she is an elected member of the National Academy of Medicine, a Master of the American College of Physicians, a Fellow of the Royal College of Physicians (London), and recipient of numerous other national and international awards.

Much of this progress has been catalyzed by a new understanding of the pathophysiology of ILD, including the role of cellular aging in pulmonary fibrosis. In this Grand Rounds, Tien Peng and Paul Wolters provide clinical updates in the management of patients with pulmonary fibrosis and describe emerging research on the role that epithelial cells and telomere dysfunction play in these often debilitating conditions.
Speakers Tien Peng, MD, is a pulmonary physician-scientist who runs the innovative Peng Lab, which uses complex murine genetic models and human specimens to investigate how fibroblasts maintain organ homeostasis, initiate disease, and drive tissue aging.

Speakers Katie Kelley, MD, is a gastrointestinal oncologist who specializes in cancers arising from the liver, including hepatocellular carcinoma, bile duct cancer (cholangiocarcinoma), and gallbladder carcinoma. Her research interests are in developing clinical trials of new treatments (including immunotherapy approaches) and combination therapies for these cancers, as well as translational research to identify molecular and genetic biomarkers of response.
Bridget Keenan, MD, PhD, is a physician-scientist who works in translational cancer research and medical oncology at the Cancer Immunotherapy Clinic. In the clinic, she cares for patients with solid organ cancers who are being treated on early phase immunotherapy trials. Her research focuses on the mechanisms of response and resistance to immunotherapy, particularly in gastrointestinal cancers.

Yet if done poorly, diagnostic errors and inequities can deeply harm patients – in fact, studies have shown that several hundred thousand patients die or suffer major harm from diagnostic mistakes each year. In this Grand Rounds, we’ll have three expert faculty explore fundamental issues in diagnosis, each seen through their specialized lenses of research, informatics, and education. We will explore the measurement of diagnostic errors, the ability to harness AI to enhance diagnostic reasoning, and the future of diagnostic excellence. This year, our department became the home of a national Coordinating Center for Diagnostic Excellence (CoDEx), funded by a $15 million grant from the Gordon and Betty Moore Foundation.
Speakers Julia Adler-Milstein, PhD, is a professor of Medicine, chief of the Division of Clinical Informatics & Digital Transformation (DoC-IT), and director of the Center for Clinical Informatics & Improvement Research (CLIIR). She is also leading CoDEx. Julia is a leading researcher in health IT policy, with a specific focus on electronic health records, interoperability, and the use of EHR data to study clinician behavior.
Andrew Auerbach, MD, MPH, is a professor of Medicine in the Division of Hospital Medicine. He is a widely recognized leader in Hospital Medicine, having authored seminal research describing effects of hospital medicine systems on patient outcomes, costs, and care quality. He leads a 13-hospital research collaborative (“HOMERuN”) focused on new discoveries in healthcare delivery models in acute care settings.
Gurpreet Dhaliwal, MD, is a clinician-educator and professor of Medicine. He is also site director of the internal medicine clerkship at the San Francisco VA Medical Center, where he teaches medical students and residents in the ED, urgent care clinic, inpatient wards, and outpatient clinic. He is a Master Clinician and an award-winning educator with a focus on the cognitive processes underlying diagnostic reasoning and clinical problem-solving and the study of diagnostic expertise.

Speakers:
Babis Andreadis, MD, MS, is a professor in the Division of Hematology and Oncology at UCSF Health and director of the Baszucki Lymphoma Therapeutic Initiative for the UCSF Helen Diller Family Comprehensive Cancer Center. His research interests include drug toxicity and genetic and patient factors behind differential responses to therapy.
Ajai Chari, MD, is also a professor in the Division of Hematology and Oncology at UCSF Health. He is also the director of the Multiple Myeloma Program. His research interests include the development of novel chemotherapy regimens, including Phase 1 and 2 national and international trials.
